This position's main purpose
is to serve as the single point entry for Community First Choice services for
individuals with an Intellectual and Developmental Disability. This position
will need to work with local Managed Care Organizations and with Health and
Human Services on a daily basis. Responsible for providing CFC
Service Coordination for the individual's on their caseload. The CFC service coordinator assists consumers in the 5-county service area in
accessing medical, social, educational and other appropriate services and
supports that will help that individual achieve a quality of life and community
participation that is acceptable to the individual. This is accomplished
through: 1) assessment, 2) service planning and coordination, 3) monitoring, and
4) crisis prevention and management.
Will answer all incoming telephone
calls about Community First Choice services for individuals with intellectual and developmental
disabilities. Meet with walk-ins requesting
CFC services. Document all initial contacts and maintain documentation of all IDD
Community First Choice intakes. Work with local Managed Care Organizations and Health
and Human Services to refer individuals to the CFC program and to then assess
their needs for the CFC program. This will include reviewing all DID and ABL
information and making referrals for testing when needed. The CFC intake
staff/CFC service coordinator will monitor the individual's ABL every five
years.
