Will oversee all CoC staff, including hiring, training, supervision, and performance evaluations. Responsible for ensuring compliance with documentation and service delivery requirements through regular audits. Will track and report key metrics to improve service effectiveness, serve as the primary point of contact for CoC services, and coordinate efforts between inpatient and outpatient providers. Duties include attending case management and hospital discharge planning meetings, overseeing crisis intervention strategies for individuals in transition, and ensuring timely completion of Home and Community-Based Services (HCBS) applications. Will lead quality improvement initiatives, resolve service challenges, and advocate for systemic improvements in continuity of care.
