Care Coordinator

Must have High school diploma or GED.  CMA licenses preferred. 

Care Coordination works closely with all disciplines to identify client needs, and to locate internal and/or community resources to address those needs.  Care Coordinators assist the consumer in navigating the referral process, both within the Andrews Center and in the community.  Care Coordinators also facilitate continuity of care by way of working with area hospitals to ensure a comprehensive after-care plan, for both new and established clients.
