In this vital role, you'll work closely with a multidisciplinary team to identify client needs and link them to the right supports-whether that's within Andrews Center or through community resources. You'll help clients navigate referrals, coordinate aftercare with hospitals, and ensure smooth transitions so individuals can focus on what matters most: their wellness and recovery.
What We're Looking For:
High school diploma or GED required
Community Health Worker license preferred
Bilingual candidates strongly encouraged to apply
Strong communication, organization, and people skills
