Bachelors degree in human service field (unofficial transcript must accompany application.) Provides Child and Adolescent Mental Health Case Management and Rehab Services.  These services include assessment, service plan development, coordinating services, monitoring, advocacy, Crisis Services, Brief Intervention, Aftercare, Wrap-Around services, skills training, parent skills training and patient and family education. Services are provided to consumers with moderate to high intensity needs. Caseload size and intensity will vary. Services will be provided in the consumer's natural environment, community and in the office setting.  Must be willing to become Wrap-Around certified.
