We are seeking a dedicated Continuity of Care Specialist to support individuals transitioning from inpatient psychiatric settings to community-based services. This position involves regular face-to-face contact with hospitalized clients, coordination of discharge planning, timely aftercare follow-up, and ongoing monitoring of services. The role requires strong collaboration with hospital staff, outpatient teams, and community partners. Candidates must be willing to travel, primarily serving Smith and Henderson Counties, with occasional support to surrounding areas. Reliable transportation is required.
