HCC Housing Specialist

Must have bachelor's degree in human service field and experience in the mental health and substance use disorder.

Must have good written and verbal communication skills and be computer literate. Must have a valid Texas Driver's License, meet insurance criteria, and be able to pass all applicable state and agency training.

HCC Housing specialist will help consumers in connecting to safe and affordable housing, while providing needed skills to maintain housing. Housing specialist will work with consumers with problems such as risk of harm to self or others, family/relationship difficulties, housing instability, unemployment, limited community involvement, limited understanding of illness, medication non-compliance, and limited independent living skills. Person will work with consumers individually and/or in small groups in order to promote recovery and independence. This position makes home visit including face to face assessments at client's home, linkages to community supportive services, and responding to needed telephone interventions. Housing specialist will maintain accurate records; HMIS and CMBHS documentation; and providing appropriate referrals and linkages to supportive services that lead to ending homelessness. Caseloads will vary in size and services will be office-based and community-based.
