Are you passionate about helping people achieve their goals and live their best lives? We’re looking for a dedicated Service Coordinator with a Bachelor’s degree in the Human Services field to join our team!
Partner with individuals to create personalized plans and help them reach their life goals
Meet face-to-face with your caseload at least once per month and stay connected via phone or in-person
Link people to community events, activities, and essential resources like Medicaid, food banks, and free/discounted medical services
Track progress, monitor satisfaction, and ensure individuals receive the support they need
Complete timely data entry in electronic health records to document services provided
Bachelor’s degree in Human Services
Strong communication skills and a passion for advocacy
Ability to connect people with community resources and monitor their progress
