Must have a bachelors degree in the human service field. Prefer two years of paid experience as a case manager in a state or federally funded Parent Case Management Program or have graduated from Partners in Policy Making. Personal experience as an immediate family member of an individual with IDD.
The main purpose of a Service Coordinator is to support and assist people with intellectual disabilities in achieving their personal goals. The service coordinator assists consumers in the 5-county service area in accessing medical, social, educational and other appropriate services and supports that will help that individual achieve a quality of life and community participation that is acceptable to the individual. This is accomplished through: 1) assessment, 2) service planning and coordination, 3) monitoring, and 4) crisis prevention and management.
