IDD Service Coordinator/Case Manager

Bachelors degree in the human service field required (unofficial transcript must accompany application).

Position Summary:

As a Service Coordinator you will:

  • Discuss what each person wants out of life and create a plan to assist them in getting as close to their goal as possible
  • Meet face to face with people on your caseload at least one time each month
  • Communicate with individuals and/or their families via phone and in person on a monthly basis
  • Link people to events and activities in the community
  • Complete data entry into an electronic health record to document what occurred during the visits you completed
  • Link people to community resources such as Medicaid services, food banks, free/discounted medical/dental services and other community services that could assist in meeting their individual needs
  • Monitor with the person to determine if they are receiving needed services, if they are making progress toward their personal outcomes, and determine if they are satisfied with the services they are receiving

**The Service Coordinator will use their personal vehicle for travel, which is reimbursable. Must have current Texas driver's license; meet insurance criteria, and ability to pass all applicable training.
