Habilitation Coordinators assist individuals living in Nursing Facilities (NF) with intellectual and developmental disabilities in achieving their personal outcomes. The Habilitation Coordinator works with Nursing facility staff, legal guardians/actively involved family members and others to provide services and supports that will help the individual achieve a quality of life and community participation that is acceptable to them. Habilitation Coordinators conduct quarterly interdisciplinary team meetings, complete assessments, monitor delivery and satisfaction of services, monitor for progress and lack of progress of personal outcomes and provide crisis prevention and management.

This position will also function as a back-up to the Enhanced Community Coordination/Diversion Coordination (ECC) and Intellectual and Development/Mental Illness (IDD/MI) PASRR Evaluators.
