Position: Transitional Case Manager TCOOMMI Program
Location: Tyler, TX

As a Transitional Case Manager, you’ll play a vital role in helping individuals transition successfully back into the community. You will:
Identify, assess, and connect clients to Andrews Center and community-based services.
Provide crisis intervention, education, and support at clients’ homes and other locations.
Collaborate with criminal justice agencies to promote understanding of mental health and intellectual disability services.
Monitor client progress and advocate for their needs to ensure continuity of care.
Travel throughout the community to meet clients where they are—literally and figuratively.
Education: Bachelor’s degree from an accredited college or university in social, behavioral, or human services.
Experience: Background in mental health and/or criminal justice preferred.
Skills: Compassionate communicator, strong problem-solver, and organized multitasker who thrives in the field.
Other: Must be willing to travel extensively to meet clients’ needs.
