Provides direct supervision for YES Waiver Facilitator who cover Henderson, Smith, Rains, Van Zandt and Wood Counties. Responsible for overall coordination of the YES Waiver Program including supervision of facilitators and contract providers, making certain documentation is precise and entered properly in CMBS, marketing services, coaching and supervision of staff, managing the YES Waiver inquiry list and hotline, supervision of wraparound meetings, provide trainings for providers, standards compliance and quality assurance, and staff recruitment, interviewing and recommendation for hiring. Covers caseloads when staff members are unavailable. This position may also may provide Child and Adolescent Mental Health Intensive Case Management and Rehab Services; as needed. These services include CNAS assessment, developing Individualized Plan of Care (IPC), coordinating services, monitoring, advocacy, Crisis Services. Services are provided to assist children and adolescents with serious emotional disturbances (SED). Caseload size and intensity will vary from 1-10. Services will be provided in the consumer's natural environment, community and/or in the office setting.
