Case Manager - Athens (FAYS Program)

Bachelor's degree in behavioral science or related field. Experience working with at-risk youth required. 

Interviews clients with problems such as personal and family adjustments, finances, employment, food, clothing, housing and physical and mental impairments to determine nature and degree of problems.  Secures information such as medical, psychological, and social factors contributing to client's situation, and evaluates these and client's capacities.  Counsel client individually, in family, or in other small groups regarding plans for meeting needs, and aids client to mobilize inner capacities and environmental resources to improve social functioning.  Helps client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and client's part in creating them. 

Will provide direct services to children ages 0-17 and their families meeting referral criteria for the FAYS program. Services will primarily be home-based and community-based.  Adjunctive services will include client intakes, case organization and management, documentation, date collection and staffing with other involved agencies.  Included additional duties as assigned.    Will perform family preservation services, crisis intervention, and social skills training for individuals and in groups.  Participates in on-call rotation.  Links families to community services and advocates for family needs.
