Healthy Community Collaborative (HCC) Program Manager 

The responsibilities of the HCC Program Manager will be to oversee the Coordinated Entry (CE) and Housing First models for Andrews Center and its partners.  This will be accomplished by ensuring compliance to all local, state, and federal rules and regulations and providing systemic and ongoing planning and coordination to meet all goals. The position will receive training pertinent to the role and participate in ongoing continuing education. The position will develop and propose processes and procedures promoting efficiency and effectiveness in achieving outcomes, act as an expert to present information to collaboratives and other entities engaged in CE and develop and maintain productive working relationships with agency staff and external service providers and partners.  Note:  This is NOT an office-based position.

QUALIFICATIONS: 

Bachelor's degree with major specialization in behavioral sciences or related field with experience in the mental health and substance use disorder.

Must have good written and verbal communication skills and be computer literate. Must have a valid Texas Driver's License, meet insurance criteria, and be able to pass all applicable state and agency training.

DUTIES AND RESPONSIBILITIES:

· Oversees CE process

· Educates organization and partners regarding CE and Housing First models

· Acts as liaison to CE partners

· CE assessment and enrollment

· HMIS assessor

· Street outreach

· Solicits safe and affordable housing opportunities

· Builds relationships with landlords

· Maintains an affordable housing list including options for people with felonies

· Supervises HCC Team

· Manages/participates on-call rotation

· Connecting consumers with safe and affordable housing

· Conflict resolution/communication with landlords and neighbors

· Budgeting/financial skills training

· Self-advocacy skills training

· Comprehensive assessments, including CE assessments and enrollments (ANSA, COPSD, VI-SPDAT, F-VI-SPDAT, and rehabilitation)

· Develops, writes, implements, and revises wellness plans in conjunction with consumer and HCC Team

· Participates in team meetings, treatment planning meetings, and internal workgroups

· Performs shift management and on-call rotation

· Refers consumers to community resources and other organizations

· Street outreach and HMIS assessor

SUPERVISORY RELATIONSHIPS:

Reports to Mental Health Division Director 

Supervises: HCC Housing Specialist, HCC Employment Specialist, HCC Mental Health Peer Support Specialist, HCC Accountant, and HCC Substance Use Counselor

SKILLS AND KNOWLEDGE:

-  Ability to communicate effectively with clients, agency staff, and legal staff involved in client's mental health treatment

-  Must be able to cope with frustration, to work non-judgmentally with clients, and to respond to changing needs of clients

-  Must be able to work both independently as well as part of treatment team

-  Ability to complete clinical documentation in a timely manner

-  Must have good phone, typing and basic computer skills

AMOUNT OF TRAVEL:  80% of the time as a result of mobile outreach, assisting clients with basic needs, performing home visits, and community training
