ACT - Case Manager

Must have a Bachelors degree in the human service field. 

The ACT Case Manager provides services to consumers with serious mental illness who are experiencing the following:  risk of harm to self or others, family/relationship difficulties, housing instability, unemployment, social isolation, limited understanding of illness, irregular use of medications, and limited independent living skills.

The ACT Case Manager provides the following services:

·  in vivo case management;

·  in vivo ongoing assessment and assistance with Activities of Daily Living;

·  comprehensive assessments/reassessments (ANSA, ACT, COPSD, and rehab assessments);

·  develops, writes, implements, and revises recovery plans in conjunction with consumer and Intensive Treatment Team (ITT);

·  participates in daily team meetings, treatment planning meetings, and ITTs;

·  performs shift management and on-call crisis intervention;

·  refers consumers to community resources and other organizations;

·  psychosocial rehabilitation;

·  supported employment;

·  supported housing.
