Children's Case Manager- Athens

Bachelor's degree with major specialization in behavioral sciences or related field.  Experience with seriously emotionally disturbed children and adolescents preferred.  Basic computer skills required.

Provides services to children and adolescents with MH diagnoses.  Training will address problems such as risk of harm, crisis intervention and resolution, substance abuse, family/relationship difficulties, limited community involvement, limited understanding of illness, medication non-compliance and limited coping skills. Works with consumers, family and collaborative providers (schools, CPS, probation) to reduce the risk of out-of-home placement and hospitalization. Helps consumer and family to modify attitudes and patterns of behavior by increasing coping, community living and parenting skills.  Refers consumers to community resources and other organizations.  Completes Uniform Assessments and Wraparound Plans (where appropriate), develops and reviews recovery plans, performs follow-up to determine quantity and quality of service provided for consumer, and monitors status of consumer's case. As a mentor will assist case managers in their facility to complete time and travel and also in a crisis situation to be contacted if needed. Also as a mentor to be giving other case manger's needed advice when called upon for assistance by other case managers in their facility.

Provides Child and Adolescent Mental Health Case Management and Rehab Services.  These services include assessment, service plan development, coordinating services, monitoring, advocacy, Crisis Services, Brief Intervention, Aftercare, skills training, parent skills training and patient and family education. Services are provided to consumers with moderate to high intensity needs. Caseload size and intensity will vary. Services will be provided in the consumer's natural environment, community and/or in the office setting
