Intensive Case Manager - Tyler

Position requires a Bachelor's degree in human service field.  Experience in the mental health and substance use disorder field preferred.  Basic computer skills required.

Individual will provide routine case management services including assessment, recovery plan development, coordination, and monitoring of services, crisis intervention, and advocacy.  Services will be provided to consumers who experience a mild to moderate level of needs intensity.  Caseloads will vary in size.  Services will be office-based and community-based. 

Individual will provide services to consumers with problems such as risk of harm to self or others, family/relationship difficulties, housing instability, unemployment, limited community involvement, limited understanding of illness, medication non-compliance, and limited independent living skills. Person will work with consumers individually and/or in small groups in order to promote recovery and independence. Person will help consumers to modify attitudes and patterns of behavior by promoting hope, personal responsibility, education, self-advocacy, and support. Case manager will refer consumers to community resources and other organizations. Case manager will complete ANSA and Rehab Assessments as well as recovery plans.
