WRAP Facilitator

Must have bachelors degree in human service field.  Experience with seriously emotionally disturbed children and adolescents preferred. Must be a Certified Wrap-Around Facilitator. [WG1] [MC2] 

This role provides Child and Adolescent Mental Health Intensive Case Management (Wraparound) and Rehab Services, including assessment, service plan development, coordination, monitoring, advocacy, crisis services, skills training, and family education. Wraparound is an evidence-based care coordination model targeting children, youth, and families with severe emotional or behavioral needs, involved with multiple child-serving systems, or at risk for removal from their home or community. The identified behavioral or emotional treatment needs may have resulted in-or are likely to result in-juvenile justice involvement, expulsion from school, displacement from home, hospitalization, residential treatment, or serious injury to self, others, or animals and should be provided in the most convenient location for the youth and his/her caregiver, including at home, school, or other community location. The position supports children and adolescents with mental health diagnoses, addressing issues like risk of harm, crisis intervention, substance abuse, family difficulties, limited community involvement, medication non-compliance, and poor coping skills. It involves collaboration with families and providers to reduce out-of-home placements and hospitalizations, improving coping, community living, and parenting skills. The role also includes referring to community resources, completing assessments, developing treatment plans, and performing follow-ups to assess service quality and consumer status.
