We are seeking a dedicated and compassionate Children's Mental Health Case Manager to provide case management and rehabilitation services to children with moderate to high-intensity mental health needs. This position works closely with youth and families in their natural environments, community settings, and office-based settings to support recovery, stability, and skill development.
Bachelor's degree in a Human Services field (required)
Experience working with seriously emotionally disturbed children
Strong communication, organizational, and advocacy skills
Ability to work independently and as part of a multidisciplinary team
Conduct assessments and develop individualized service plans
Coordinate and monitor mental health and community-based services
Provide advocacy and linkage to resources
Deliver crisis services, brief interventions, and aftercare support
Provide skills training, parent skills training, and family education
Maintain documentation in accordance with program and regulatory standards
Manage a caseload with varying levels of intensity
Services provided in clients' homes, community settings, and office locations
Caseload size and intensity will vary based on client needs
